EPIDEMIOLOGIAL STUDY OF PARAGONIMIASIS IN YANGJOO-GOON, KYUNGGI-DO

Abstract
Yangjoo-Goon Is Iocated in the northern part of Kyunggi-Do and borders on Pochun-Goon and Ka-pyung-Goon, with Pajoo-Goon on north west, with Kyoyang-Goon on the south west, with Kwangjoo-Goon on the south and with Yangpyung-Goon on the east south. Yangjoo-Goon is relatively mountainous and contains sixteen Myuns, . Euijungboo,Jangheung, Kwangjuk. Baeksuk, Ihidam, Jinkun, Byulnae, Hoichun , Jinjup, Joonae, Nam, Tunhyun, Mikeum, Wahoo, Hwado and Soodong-Myun. EachMyun has about 10,000 population and has 5 to10 Ris except for Euijungboo,
This study was directed to this Goon as a part of the program of studying directly making a trip to each Myun in every Goon in Kyunggi-Do as finding out the districts which have the crayfish and crabs having Paragonimus metacercaria is urgent because many children in rural areas eat raw and slightly roasted crayfish and craba and there is a traditional custom in Korea of taking raw crayfish juice for the treatment of measles. producing many public health problem.
Among sixteen Myuns in Yangjoo-Goon, Euijung-boo is excluded because it is considered to be notin fested by the Paragonimus westermani. One primary school from each Myun in the other fifteen Myuns was selected and the pupils in older age groups were tested for this study being asked about their custom of eating raw inadequately cooked crayfish and crabs and their environmental situation regarding drinking water, and testing them with Paragonimus antigen.
The crayfish caught from these Myuns were examined for Paragonimus metacercaria
The children ate raw crayfish were ranged from none in Nam-Myun to 42.3% in Jinkun-Myun, many male children and few female children In most of the Myuns in this Goon ate raw crayfish, the children ate poorly cooked crayfish were ranged from 39.3% in Nam-Myun to 95.7% in Hwado-Myun. Crabs were rare except for several Myuns located along the North Han River and the Han River where some children ate soysouce soakedor inadequately cooked crabs. Such eating habits among children in this Goon are suggestive of a large infestation and of a continuation of Parago-nimiasis from early childhood. In environmental situations, those who drink the water of streams the water directly.
Those children who drink such water were ranged from 15.2% in Nam-Myun to 69.4% in the pupils of Hyunam Primary School in Kwangjuk-Myun.
The districts showing higher¡¤ positive rate with Paragonimus skin tests have infested crayfish with paragonimus metacercaria in this Goon and the districts disclosed as having higher incidence of Paragonimiasis from this study were Sang-bookok-Ri, in Jangheuns-Myun, Biam I, II Ris, In Kwang Juk-Myun, Hongjook-Ri, in Baeksuk-Myun, Palhyun Ri, Jinkun-Myun and Woibang Ri, Ipsuk-Ri, Woosoo-Ri, Songchun-Ri in Soodong-Myun. All of those are located at the deep mountainous areas, where the higher percentage of the people drink the water of streams and from shallow wells.
This Goon is relatively mountainous and in the many valleys in the deep mountains, the first intermediate host, snails, the second intermediate host, crayfish, propagate actively and are produced in enoumous numbers. Crabs are present in the limited season from August to November and in the limited areas located along the big river and are not likely to be a second intermediate host of Paragonimiasis in this Goon. The infestation rate of crayfish with Parahonimus metacercaria was found to be as follows : 0.9% in Jangheung-Myun, 1.1% in Kwangjuk-Myun, 1.2% in Baeksuk-Myun, 0.3% in Joonae-Myun, and 4.3% in soodong-Myun respectively.
The custom of taking raw crayfish juice for the treatment of measles is also prevalent in this Goon which may give many harmful effects to growing children. I wish the public health authorities and the school authorities in this Goon would strive to eradicate this bad custom.
